Plasma cell neoplasms

Plasma cells originate from B lymphocytes. B cells produced from bone marrow. Generally, when foreign pathogens like bacteria and viruses try to enter, they change into plasma cells. Plasma cell neoplasms are conditions where B cells make abnormal plasma cells. These are abnormal ones since they produce not needed M protein. These antibodies cause blood cells to thicken. There is also deposition of such proteins in the bone marrow.  These will eventually damage major organs like the liver and kidneys.

These plasma cell neoplasms can be benign (not cancerous) and malignant (cancerous forms)

Monoclonal gammopathy of undetermined significance (MGUS) can eventually become cancerous. It is generally not of cancerous type. The following types of plasma cell neoplasms are cancerous

  • Lymphoplasmacytic lymphoma (also called Waldenstrom macroglobulinemia)

Monoclonal gammopathy of undetermined significance (MGUS)

This type has 10% of cells in bone marrow containing abnormal cells. Hence no tumor development. These abnormal M proteins present in routine blood or urine tests. Thus, they are specifically circulating in body fluids. For most of the patients, the number of M proteins is the same. Therefore it shows no signs of symptoms or disorder.

However, in certain other patients due to other underlying problems, the scenario is aggravating. They will develop amyloidosis, major organ damage such as liver, heart, and kidney damage. MGUS will become cancerous like multiple myeloma, lymphoplasmacytic lymphoma, or chronic lymphocytic leukemia. These can be in benign or metastatic forms.

Plasmacytoma

In this kind of plasma cell neoplasm, the plasma cells become localized to form a tumor. However, mostly it can be cured. There are two plasmacytoma categories – isolated and extramedullary plasmacytoma.

  1. In the case of isolated bone plasmacytoma, there are different cases there are no signs of cancer. Less than 10% of these bone marrow cells are made of abnormal plasma cells.
